Clicking Title Bar 4 times causes Window to minimize and open.

This only happens if you have double tap to minimize. Of course this isn't a bad thing. You can double tap again (in the 0.5 second frame) to save the window.

This seems to happen only with some apps...

I can reproduce it in the Finder... but I don't see that this is really a problem. I had to work hard to get the four clicks to all be fast enough.

Is this really a problem? How often does it come up, and does anything bad come of it?

Yeah, what's the issue? I mean, if you want the window minimized, then you will only click twice. And if you wanted it minized and then changed your mind, it seems like this is actually a nice feature.
